The input modal that displays when running a parameterized pipeline fills the entire width of the screen.

Repro Steps:

1. Navigate to the ATH job on Dogfood: https://ci.blueocean.io/blue/organizations/jenkins/ATH-Jenkinsfile/branches
2. Run any of the branches (e.g. "master")

Expected Behavior:

1. Dialog has a maximum width of 464px
2. Dialog should scroll within itself if the content area
3. Dialog max-height 90%

Actual Behavior:

1. Dialogs occupies 90% of screen width.
